Transcript Requests

Please print the form and mail it or fax it to 410-888-9278. The transcript fee is $5.00 per official transcript for most states. Currently, transcript fees for CA, WZ, AZ, and NM are $50.00. Be sure to include any forms required by your state to be included with the transcript. We recommend double checking with your state about current requirements prior to requesting transcripts. Please allow a week to ten days for processing requests. We do not email transcripts.

Tai Sophia Community Forum

We are excited to announce the most recent addition to Tai Sophia’s online endeavors. The Tai Sophia Community Forum, located at http://forums.tai.edu, is a new online forum that serves as a digital bulletin board for alumni, students, staff, and faculty. It’s a very effective way to stay in touch with each other, and engage in a myriad of discussions.

HIPPA Compliance Forms

These documents were developed to enable the acupuncture and complementary medicine clinics of Tai Sophia Institute to meet the privacy requirements under the Health Insurance Portability and Accountability Act of 1966 (HIPAA).

In support of our practicing alumni, we have provided online copies for your reference and usage for implementing in your office. These copies are provided only as an illustration of how compliance is possible. No part of these documents may be reproduced, stored in a retrieval system, or transmitted in any form by any means, electronic, mechanical, photocopying, recording, or otherwise, without written permission of Tai Sophia Institute.

You may purchase a CD-ROM from the Meeting Point Bookstore at Tai Sophia Institute that provides digital copies of these documents. Your purchase of the CD-ROM entitles you to adapt these documents exclusively for your own use.

We ask only that you acknowledge Tai Sophia Institute as the originator of the documents and print our copyright on all documents. The permission to use these documents extends only to you, and only if you purchase the CD-ROM. Any other use constitutes infringement on our copyright and will be prosecuted to the fullest extent permissible by law.
